Judging by number of deaths it would be Havel. I died to him nearly 100 times in the 24 hours it took me to get past the Undead Parish on my first playthrough.
Once I got the past the steep learning curve the rest of the game was relatively easy

Havel has never killed me. Actually, I've never fought him. I have always killed him through the door.
