For anyone who says Dark souls/Demon's Souls
Call of Duty 3 and World at War on Veteran mode. Good luck rushing past the monster closet with 5 grenade indicators + Grossly bullshit AI. In 2006, Call of Duty 3 took the award for Hardest game, due to the AI's Psychic abilities and grenades. WaW in the Japanese Campaign wasn't much better.
That's in recent memory though. Max Payne 3's New York minute on Hardcore.
Old Style though would be Contra.
