There have been many but one of the few that comes to mind is one of those rare games I haven't actually beaten because it was simply really f*cking difficult to me, is Mortal Kombat Sub Zero Mythologies. I couldn't get past the second level. Lol that fight Fujin was a real bitch.

The hardest games for me are the ones that made me think critically like outlast or maybe even silent hills series. If not that defiantly Super Mario 64 with its lame camera angles or Command and Conquer on brutal mode.
