Xbox One player here,
I've been having this issue for SO LONG now! I can't seem to start a mission, patrol, or even the raid - and this only happens on MOON. I always have connection issues on Moon. It doesn't happen on any other PvE worlds like Earth or on the raid on Venus. It doesn't even happen on PvP. I always have flawless connections on those activities. So here's me detailed report:
Patrol: If I start a patrol on Moon, I'd go around farming and whatnot, exiting and entering different areas on Moon. First, the sounds start to come in snippets, and not at the right time either. Then players start to go invisible(like the Hunter ability) with their levels always being at 0. At this point, if a special event were to start, the little blue and orange banner sign that shows that it has started would appear, but the text would be GONE(remember, the sound is still lagging). At that point I would just leave after being so frustrated with it.
Missions If I start a story mission or strike, it would boot up just fine. It would move along well until I hit a certain point. A loading circle pops up in the circle with the word "Loading". While in this state, my guardian's legs would remain frozen but his upper body could spin around(I could turn around and see my butt). It would stay this way until it lags me out or I leave. I think when it does that, it's where the game tries to separate the fireteam from the other players who are not in your fireteam.
Crota's End For the moon raid, I'd group up with some players and make a fireteam. We'd start it up, and as soon as the ships leave orbit and the screen goes black, the sounds start to come in snippets(again, lagging) and the screen STAYS black. This happens for ALL the players in the fireteam. It continues to happen for about a minute or two and then ultimately just crashes. When I join an active raid session, my fireteam has no problems, and they tell me that I am in the game, but they can't see me at all. At the same time, my sound is lagging again and my screen is black. Same thing happens when I start a game by myself.
Why does this happen? NAT was never an issue, but even if it was already open, I went to my routers settings and opened up all the ports that I needed. But this did NOT help. The same things continue to happen, and only on Moon. If play Destiny on my Xbox 360, and I can run ALL the activities there with no issues whatsoever. I was so eager to play the moon raid on new gen, but I can't. Also, no matter WHAT other game I play on my Xbox One, whether it be AW or ACreed, I have no connection issues. Only on Destiny, and only on Moon. What do I do??? What do you suggest? I know we can BOTH handle this, Bungie. We can both kick this problem in the ass and send it into oblivion. Hopefully you can actually see this and I hope this can help bring you another step closer to finding an answer to these connection problems.
Bungie, Guardians, let's kick some ass

UPDATE: After most of the problems occur, the end result is almost always Destiny freezing - not crashing and restarting. Even after restarting Destiny, the problem still occurs.
