So I recently noticed this and I am sure its happened. This happened during crucible mode-clash. At first when it happened I had my shotgun out and I pulled the trigger to fire, and nothing happened, no shot, nothing, so I thought I may have not pulled it hard enough. The second time it happened I definitely pulled the trigger hard enough I made sure I was after the first time, yet no fire, ammo clip still full. (BTW this wasn't one of those things where I shot someone while dying and it didn't get the shot off, I was alive.) It has happened several times with only the shotgun. The other issue was jumping. I pressed jump to get over a chasm, yet when I pressed A again to blink across nothing happened causing me to fall to my death, this also happened several times. My controller has been at full battery and I'm not playing at some weird range where it would go out.
