How do we know that Pyramid Head isn't a girl?

I mean we never heard it speak, and plus it wears a dress. How do people just "assume" that it's a guy? I mean even the muscles, it could just be one of those chicks that work out!

After finding out that the Butchers in Dark Souls (pic related) are actually female, I am no longer sure of any video game enemy's gender.
