Was doing a run of Contest of Elders with my friends, and when we all ended up dying and no revive tokens were available to use, the game said "Finding Spawn Location", but never letting us respawn. I'm thinking, because you're intended to be returned to orbit if everyone dies and no revive tokens are available, that it's being buggy and ironically keeping you trapped in the mode instead of sending you to orbit.
