Access Override Triumph wont unlock and puzzles wont appear
My friends have completed Spiders quest, but the triumph Access Override never unlocked and the first two puzzles wont appear. Strangely puzzle 3 showed up in one run. Anyone else seen this?

Finished the quest, started a new dungeon run by resetting the checkpoint, and triumph will not appear, still ran to see, first puzzle showed up in the heart room after 1st encounter, but the other 2 did not. Going to see what a weekly reset does, but the triumphs don't show up for me or the puzzles.
