Icebreaker catalyst
After getting icebreaker and doing all the puzzles. I didn't get the catalyst After shooting all the crystals and entering the code. The other people in my fireteam didn't get it either

I had this issue. I had to relaunch with another fireteam leader. And also make sure to destroy crystals with ice breaker (and not with abilities like i did lol) and to activate all the buttons to activate the chest where you enter the codes. Try with another character as well. Its hard to understand the bug but gry different things as a workaround. The fireteam leader has to be on this particular step tough
