Shoot crystals with icebreaker…only the people who have completed both after quests should do this and are up to the crystal breaking step. Second shoot the four small number panels whilst going to the chest. At the chest have one person on the crystal step enter the code and then open the chest together. Everyone in my team, two of us on the last step and one who hadn’t even completed the two after quests received the catalyst. Our catalysts were complete whilst the our other team mate had kills to do before his would be finished. He would still have to do both weapon and armor focusing steps if he ever wanted those.

I had this issue. I had to relaunch with another fireteam leader. And also make sure to destroy crystals with ice breaker (and not with abilities like i did lol) and to activate all the buttons to activate the chest where you enter the codes. Try with another character as well. Its hard to understand the bug but gry different things as a workaround. The fireteam leader has to be on this particular step tough
