Basically, while on my solo flawless run, the boss decided to re-form when I reached the "last stand" mechanic, resulting in me being softlocked, since the boss never actually reformed, the mini-servitors were left there to be killed but the game still registered the end of the damage phase, giving him back the immunity shield.
