Breech activity not working properly
I tried six times on each character to complete the 3 breech activity, as soon as I passed two, it reset itself, what's going on?

Because there are 3 tiers of rewards. When you get 3/3 it goes up a tier. Tier 1 is powerfull. Tier 2 is powerfull. Tier 3 is exotic.
