Khvostov quest bugged?
I have all 8 visions and defeated 9 bosses but only show 16 motes???

You havent defeated all the bosses then. The triumphs associated with defeating the bosses are however bugged, they can be completed by defeating any 3 bosses regardless of whether or not they are the same bosses. The best thing to do will be to go back and kill all the bosses again, from my experience the ones youve done wont show up first.
