How Petra lost her eye remains an unsolved yet interesting mystery, which after some digging has left me with sone theories.
Firstly we know Petra had both her eyes during her days as the Queen's Emissary, the Honored (https://www.ishtar-collective.net/entries/honored) lore tab says; "Petra cannot see anything but her own distorted reflection in her polished sabatons. She closes her eyes." Now, one could go the route of symbolism here, and say that 'because she's looking at her distorted reflection, from her perspective she looks like she still has both her eyes'. Frankly the use of the plural "eyes" is enough for me to say that Petra had both eyes before the events in the House of Wolves. If I must go further, it worth noting Petra didn't have any of her scars while she was the Queen’s Emissary, (none that could be seen at least) that would indicate she'd sustained an injury prior to that point. Granted at this time Petra didn't have a unique character model, and was wearing a helmet but still. So, we can rule out the Reef Wars.
One theory I have is that Petra lost her eye during the Wolf Rebellion, when the Queensbreakers made their assassination attempt on Mara, the event that kinda set off the House of Wolves DLC. But if Petra had lost her eye in defense of her Queen, I don't think she'd refuse to talk about it. If anything, I could see Petra viewing it as a badge of pride and she'd probably have mentioned it during HoW. Furthermore, and this is stretching, Petra's injury seems very deliberate given her scars, meaning I don't think she was shot. Which wouldn't line up with what happened during the assassination attempt. Realistically facial lacerations tend to heal fairly well and usually have to be EXTREMELY SEVERE to actually result in permanent visible scarring, and the scarring just doesn't line up with being shot in head (Granted we are in a sci-fi civilization with sci-fi weapons as well as medicine).
Which leaves my current theory, that a Guardian could be responsible for Petra's lost eye. Now, those familiar with Petra's past would know, she gave an order which resulted in the final deaths of nine Guardians and their Ghosts, this is why Petra was demoted to Queen’s Emissary and exiled to the Tower. Guardians as we have seen, aren't the best at handling loss (and to fittingly quote Osana Sov in the Marasenna "There is no weregild for the death of an immortal", let alone nine of them). I came across something kinda interesting on the Royal Executioner (https://www.ishtar-collective.net/entries/royal-executioner) lore tab: "Though they appeared civilized, the Lightbearers were not far removed from the utter barbarism of their Dark Age. She sensed that same savagery still seething just below the surface."
I find this an odd remark coming from Petra given the fact she's been working with and around Guardians for years, probably longer than most Reef Awoken. Granted Guardians silly and frivolous tendencies are very canonical, and Petra was there when Uldren died. But she's just as likely to have shot Uldren, if not more likely, than the player. Which brings me to believe a grief ridden Guardian might have taken Petra's eye as an act of vengeance for their deceased comrades. I can imagine a Guardian taking the eye for an eye literally, or straight up attempting to murder Petra and almost succeeding only for her to escape. Which would also explain why Petra won't talk about it.
The only problem with this theory is it's difficult to place when this would've happened. As it would've happened after Petra became the Queen’s Wrath (as previously established), which would've further destablized relations between the Reef and the Tower. However, it's worth noting that Mara does seem to poccess the capacity to silence people and Guardians alike while keeping her hands clean in the process, and Uldren was Petra's friend as well as Commander if the Crows. Also the Queen’s Wrath is far from a sitting duck even without her left eye. So it's entirely possible that if a Guardian went as far as to attempt to exact vengence on Petra, whoever was involved probably never lived to tell about it.
Still I think its an interesting question, what do you guys think?
