It seems to be most common on slug shotguns, sniper rifle shots, and fusion rifles (aka 99% of what the Crucible is primarying 24/7). You slide, the shield comes up in front of you, and I think that you reflect the projectiles into yourself as you're moving forward, thus killing yourself from your own reflected projectiles.
