I have seen other posts in this forum and Reddit about this, and can confirm this is a SOFTWARE ISSUE and nothing to do with my PS4 (regular 1TB model, not Pro).
It’s the BSOD that only occurs in this instance:
1. Game runs normally.
2. PS4 is put into REST MODE.
3. On restart, game boots to spinning logos before character selection screen.
4. BSOD with Sony Error CE-34878-0 (general game crash error).
I have:
1. Moved D2 from internal to SSD and back.
2. Deleted and reinstalled twice.
3. Cleared cache.
4. Reinitialised console.
5. Rebuilt database.
6. Cleaned inside PS4.
7. Replaced PSU thermal paste in case the console was overheating.
I cannot reproduce this error with any other games, and D2 did not crash until after the first season patch (Season of the Lost).
The only “workaround” is to close the application each time I finish playing.
This is now getting very tedious and isn’t getting enough traction for Bungie to look into it.
10/12 Update: cleaned PS4 and replaced thermal paste. Error still occurs on restart from shut down mode. I am still
convinced this is a software issue.
10/13: After the latest update 3.3.1 (large update for Festival of the Lost), the game has ceased to crash before the character select screen. PROBLEM SOLVED.

I have a similar issue, although my crashes happen at more than launch, and do not seem entirely connected to putting the game into rest mode. The recent patch has not helped. But I'm glad to see that I'm totally screwed. :p

Try safe mode booting.. then from the options presented choose rebuild database, this should fix the issues
