Prophecy Cube Room Broken
In the Cube room, there are times when, after dunking motes and stepping on the central plate, it will raise you to the center cube and drop you straight back into the same room, preventing progress of the encounter.

same here. has happened a few times to different players in my team over the last few weeks. you hit your head on the cube and arent teleported into the next room
