It still happens on different occasions and I managed to reproduce one occasion it happens 100% of the time in both PVE and PVP, even in menus too (for me at least). It also happens randomly for me many times during PvP games. I hope this issue gets acknowledged and investigated so it can get fixed in the future patches.
