It's the void orbs. I don't know why, but trust me, that's it. I've seen it happen in the Sundial with the encounter that has the same orbs. The only way to hopefully get around it is to not be the guy picking them up.

That would explain the Pit of Heresy, but what about the Alter of Sorrows? No orbs to pick up during that event

Could be the swords then. It has something to do with picking up objects.
