Happens to me on Xbox One. Since Warmind update, my Elite controller loses connection with Xbox often. What’s odd, is that it is tied with certain events. I notice that with periods of heavy in-game activity the controller will get really bad latency, but what will really bake your noodle is this: I received a new emote; when I “moused” over the emote wheel to press Y to open, once the cursor scrolled onto the emote box, the controller light flashed, rebooting and reconnecting. Odd, I said, so I tried again. Same results. And again. And again. And again. Reliable. In-game activity was correlating 1:1 with controller reboots/disconnects.

This is a known issue because the XB1 runs so warm and the CPU and BT modules are VERY close to one another. I would first try a regular controller to see if it is the problem with the bluetooth in your controller or the bluetooth on the motherboard of the XB1. That should rule that out. If a regular controller works fine and does not experience the same problem, then your BT on your controller is going wonky. Also, make sure you update to the latest firmware on your controller in the XB1 settings, i have seen this happen also due to this issue. If it occurs with another controller, the problem is starting to occur due to heat build up in your console and possibly a failing BT module. I have had to send back one of my XB1's (original black boxes) twice now due to failing BT modules.
