Example 2 would be strike specific loot. How was that left out? Seriously, when it was added into D1 it was very very popular. During the development of D2, this could have been added fairly easily IMHO if it was not on the radar. And if the D2 dev team had no idea this had become a great thing in D1, there is serious problems at Bungie, especially when their leader Luke Smith knew about it...
