Aksis P2 Challenge No Exotic Rewards
We did the challenge to complete P2 on Aksis, some party members got AoT ornaments, one got the sparrow, but none of us got any exotic weapons. When we pushed him to final damage phase, it was on the 1st teleport of the 3rd bomb phase. He teleported to the right, we pushed him, and for some reason, before he teleported to his platform and did his final density, he teleported to middle, but before we could supercharge and slam on his back, he teleported to his platform. Is there any reason that would have prevented us from completing challenge, although we did not have the time to complete the steps?

That's exactly why it didn't work, and why, on the runs that I did, we waited to get him to his final stand until his third teleport. I know it sucks that it screwed you like that, but the challenge mechanics counted that as a teleport without a supercharge, and therefore failed the challenge.
