Okay so I was watching Star Trek: The Original Series, and it was the episode "where no man has gone before" and at the part where Lt. Mitchell makes Captain Kirk's grave appear the grave stone reads James R. Kirk, and it is a well established fact that his name is James T. Kirk.
Now it is true this episode never aired as the pilot episode but it just goes to show with the Sci-fi empire Star Trek became, how little they planned ahead for it become a huge sensation

Yeah, it's a well established Star Trek blunder. I don't know your age, but it's cool whenever people give that old series a chance. That pilot is one of my favs. The eyes were so creepy and I loved how Lockwood played Gary Mitchell. That pilot was made 50 years ago this year. Amazing.
