You are taking it out of context.
Shoulder charge requires a pre-run up, a semi-planned hit must be organized.. The only issue is the fact that you can run into a wall. Other than that, steps could be taken to avoid something that you can counter. Shotgun? Distance?
A scatter grenade is usually set up and can be escaped from. Never been killed by a directly thrown scatter nade, never will. You can also escape from them.
A Firebolt cannot OHKO with only the burn. You still have to work to get their health down low. It is still however broken. If you throw it mid-shooting, you're probably dead anyway.
The point is that in most situations it is an insta win with no/little pre-planning required. Two guys see each other, one of them throws both the smoke and vortex nade. It's over for the other guy. He can't do anything, he's lost. The best he can ensure is a trade. The only way to act against it is to keep distance. But with the other user having ShadeStep.. Not the easiest.

Who cares? It's still overpowered as it is. An insta win button is foolish. It ruins your vision and disables your grenades (Been a while, can't remember) The excuse is redundant when it was used for other classes and didn't work. The thing is, the other people have their abilities before dying of they do happen to do so. They still retain many things from what smoke takes away.
