Today, I got my Chaperone up to 310 attack; so I decided to give it to my Hunter, who needs a good shotgun. My Hunter also has an Omolon Kumakatok HC4 Hand Cannon with Luck in the Chamber and Firefly; a nice substitute for my Ace of Spades when the Chaperone is occupying my Exotic slot.
Except... the Chaperone still has that glitch where the Roadborn perk triggers with every precision kill regardless of the weapon you're using, and it seems to be causing a glitch with the Firefly perk.
What happens is, when the Firefly perk procs, it triggers two explosions- one centered on the enemy you killed, and one centered on your character. At the same time, it triggers the Roadborn perk on the Chaperone.
When stringing together multiple headshots with the Firefly perk on your weapon the Chaperone in your pocket, it causes the entire game to freeze. To test it, I tried swapping the Firefly perk for "Danger Close", and I stopped having the problem. I really hope Bungie fixes this; I like having Firefly, but I can't use it when I have the Chaperone equipped if it's constantly causing the game to crash.
Has anyone else had this problem?

I have a very similar issue. There is a 50% chance that if I use "Tread Upon Star" (which has "FireFly" on all the time), on the Dreadnaught patrol with my Titan, the game will freeze. I can switch back to the Desktop and use my Xbox One as normal, but Destiny is frozen solid, requiring me to "quit" that app.. The only other perks on this Scout Rifle are "Zen Moment" and one of these selectable "Oiled Frame", "Explosive Rounds" and "Demotion" perks. It has happened with any combination of these perks on or off and it only happens after a crticial head shot with explosion. This appears to only happen for my Titan and only on the Dreadnaught. My Hunter and Warlock both have Scouts or Pulse rifles with "FireFly" and I have never seen it happen. I have never have my Titan freeze up with "Tread Upon Star" on any other location except the Dreadnaught on patrol.

The Firefly perk itself is what does it. The Vanity from FWC (whatever the current HC is) has explosive rounds and Firefly which causes it to crash very hard.

That has nothing to do with the chaperone.. The game crashes when firefly and luck in the chamber proc on the same bullet.. Bungie is well aware of this crash and it's in their help forum somewhere..

Firefly causes crashes in a variety of situations. It's pretty well document, but they've never addressed it.
