Proof via saved gameplay, if you don't like reading, watch it (skip to 2:08 and watch the horror unfold) and cringe after rewatching everytime.
Played lvl 28 PoE because the farm is real for motes, SCs, planet mats, armor mats and weapon parts, etc. a bunch of stuff. But anyway this might have been a glitch but damn does it really make you realize that what most of us do all the time when we try to drop to the Queen's most excellent Barrows can have dire consequences.
So we (me and my matchmade randos) beat the round 5 Ultra as per the usual and head back to the airlock as Variks commands us to. The shifting floor panel on the floor opens meaning time to open some chests. So I do what me and my friends and everyone else I've played with do, start jumping off the cliff like your parents told you not to do. My fall ends in death, but where I respawn is less than disheartening.
So here I am back in the round 5 hive room where we slayed the Ultra. The door back to the airlock opens and I enter into the hallway only to be further disappointed. I cant get back into the airlock thus can't drop down to the treasure room, thus I cannot receive the rewards from the chests...the salt was just too real.
On your end Bungie...I don't know if I was too close to the wall thus making the game think I died in one of the wave rooms thus making my ghost appear to be inside one thus making me respawn in said room. However, I know coding is hard and this isn't such a priority to fix so instead I'll advise my fellow guardians for future reference.
So as a PSA of sorts, I know us Guardians out there love testing fate and dropping low to the treasure filled floor after a PoE run but alas, save your jump,lift,glide or make them count when descending and don't make the same mistake I did.
P.S. Honestly wouldn't have even cared or posted this if a ship dropped for me in the post-activity reward screen so please increase that RNG drop rate please as well Bungie. Thanks :)

This has been a known issue in POE since inception which is why it has been advised to jump CAREFULLY down. if I remember correctly it was even mentioned in a HoW weekly update. It happens, there is nothing you can do sadly but shoot at that room in frustration

Update: Played another run right after and low and behold, another unfortunate soul fell and died before reaching the chest room and respawned to meet the same fate. Though he did get a legendary engram from the activity rewards screen so I think he's alright.
