I got it inside the place you go to get the farmer's dead daughter's locket from some raiders.
Had to lock pick a novice level door downstairs inside the base. Right when you open the door, a legendary radroach jumps at you. Kill it to get the gear. Further into the room there is a dead body and a case. The case has a key in it. The key unlocks a door close to when you enter the building.
Inside is a mini nuke and some other goodies.
The location is near the car junkyard close to Sanctuary, can't remember the name atm.

Apparently it is a random encounter though, none of my other friends found it in the same place, or at all.

"Novice" Door? The hell? It was a "Master" level for me, and I had 6 Intelligence!

The door that when you opened it had a dead guy and a tool box with a key in it? Novice for me, maybe it's random.

Are you talking about the door you have to hack with a terminal, or is it a door you have to lockpick?

Lockpick the door downstairs. You get a key down there that unlocks the master door upstairs.
