The thing is, you don't get moulding shards on hard (unless you didn't do notmal). So if the chest doesn't drop you a hard mode loot drop, i.e. a ghost, it will appear as if you've gotten nothing, when in reality, you just didn't get a hard mode drop, and normal was already done. The lack of materials for upgrading basically means, gear or nothing.
