Did the same thing to me...had to run it three times to complete it.. This is what worked for me.. make sure that you kill your knight with whatever element sword matches him and the quest (I actually killed each knight with the type of element sword that matched-- if you're running 3x chars and can do that) then just take him down (first) and darkblade in the same 30-- it will unlock.

Not to my knowledge.. After everything you go through to get the sword I wasn't taking any chances on running that thing a 4th time..
