Fun question. It would be interesting to know. It's probably one of two things:
(1) Queen's Guard mask for a hunter from the original Queen's Bounty event
(2) One of the pumpkin heads from Halloween
As for weapons, it's probably the Praetorian Foil, but I have no idea.
