[Solved] Defective Disc Replacement?

I purchased my copy of Destiny for the Xbox One in December. Recently the disc started making odd noises and taking multiple inserts to get the Xbox to read the disk. Upon closer inspection, the middle of the disk is crumbling apart. I am assuming this is causing the drive to have a difficult time spinning the disk. The game has never been anywhere other than the drive and the case it came with, and it's the first time I've ever seen a disk do this. I am assuming that it is a defect in the disk itself. Is there any sort of replacement plan for the disks where I can trade in the disk for a new one for a small fee? I know some movie companies have similar programs for blu-rays. I just wanted to see if I was SOL and needed to buy a new game (would probably just wait for expansion), or if there is a replacement program where I could replace it cheaply. I also do not know if this is the correct forum for this or not.
