[spoiler]This is a tough one, but i think for me it's when Joel's daughter (i think her name was Sarah) died at the beginning of the Last Of Us...i found that scene to be really heartbreaking.[/spoiler]

[spoiler]I think it would be when Riku summoned his strength to hold back Ansem who was using his body and trying to kill Kairi.

The ending of BioShock Burial At Sea Episode 2. [spoiler]Having journeyed with Elizabeth all through Infinite and the two DLCs, and watching her slowly die with Sally and "La Vi En Rose" playing was utterly heartbreaking. Nothing in a video game has come close to replicating the feelings I had at the end of that DLC.[/spoiler]

The Last of Us. Just, the whole game. From start to finish, you get terrified, feel happy, feel sad, and everything in between just as the characters on screen do. It's so freaking real. All the feels.
