Yes but they inject a weakened or disabled form of the virus. The reason you feel sick is because your Immune System is mounting an attack on the virus, so it does what it usually does when the body gets sick (accelerates fever growth because it helps to mobilize the Immune System and make the response more efficient). I'm pretty sure your nose congests so that its harder for more bacteria to get in the body.
