As for the hydras and ogres, it has to be regular (red health bar) enemies. Majors (yellow health bar) doesn't count.

That seems silly as ultra/majors are harder to kill.

No what really is silly is all you ever see ogres and hydras as is yellow bars, gotta farm story missions but I feel like it stops counting after so many in a row!
