It's random. This kind of stuff was being posted in the borderlands 2 forums ages ago. Your brain will always try to make sense of things and likes looking for patterns, there aren't any. I do believe the RNG system is weighted where item A might only have a 0.00005% chance of dropping and item B might have 5% chance of dropping, but its still random!
