So I'm watching through halo lore videos in my free time and I've come across one that includes the gravemind talking with chief, the arbiter and the monitor. The monitor mentions that that particular installation has a success rate of 'X' trillion simulated and one actual.
So that installation had been fired in the past? The only novel I've read is fall of reach which didn't touch on any of that I don't think.
I was always under the impression that the proper firing of a halo destroyed the halo. That's obviously not right.
So my question is, how many halos have been fired in the past?

All the Halos were fired once before from the Ark by the Forerunners. The only Halo to explode or whatever from a firing was 04 because it was unfinished.

The rings fire the pulse that destroys sentient life but do not destroy themselves, that only occurred in Halo 3, when the master chief purposely fired the premature Halo ring being made on the Ark because it would blow instead of firing, destroying all remaining flood. Also in Halo CE Instalation 04 was destroyed by the Pillar Of Autumns fusion reactors to also destroy all local flood infestation. The rings that are around now in the Halo universe have been fired once and that was by the ISO Didact as a last resort to destroy the flood 100 000 years ago, that firing killed all life and flood organisms in the galaxy, apart from the species, flood specimens and certain forerunners that were safe on the rings and forerunner shield worlds eg The Didact on Requiem.

12 Rings were originally created. Later, 6 more smaller but more powerful Rings were created. 11 of the 18 were destroyed, and the remaining 7 were activated.

We know of at least 5 Halo firings. > Installation 07 fired over Charum Hakkor, destroying the Precursor constructs there and releasing he Timeless One from its prison. > Installation 07 fired over Janur Qom, the San'Shyuum (Prophet) homeworld on the order of the Master Builder to quell a rebellion. > Omega Halo, last of the 30,000km diameter Halos, fired at Path Kethona to slow the approach of the Star Roads into the galaxy. > The Halo firing itself, wiping out life in the galaxy. > Installation 04B in Halo 3, the ring was unfinished which is why it fell apart. Halos do not generally self-destruct when fired.

7 have been fired once, towards the end of the Forerunner-Flood war. Firing doesn't destroy the ring, that only happened in Halo 3 because it wasn't fully constructed yet. There are 5 other rings... I don't recall what happened to them, it's been a while. I do know a few rings were fired on a localised basis to target a single planet on a couple of occasions.

I think they were destroyed by the flood in the battle between the flood and forerunners over the Ark.
