I think this is easy. It's red death. I have never seen it drop and I have dozens of people on my friends list who play destiny and none of them have it. The people who I've played with who did have it, bought it from xur.

I own 3 red death, and the last 2 I received were from raid rewards, which allowed me to dismantle the 2 lower level (300 attack) copies I had bought from Xur long ago. Saved me 14k in glimmer from having to upgrade them. ;)
